Meet The Providers

Sarah Fontaine, MSN, APRN, FNP-C

In 2013, Sarah graduated from University of Saint Joseph in West Hartford, Connecticut with a bachelors in nursing and minor in biology. She has practiced as a nurse in skilled nursing facilities, homecare, hospice and palliative care, and in hospital. She received her certification in hospice and palliative care in 2017. Sarah went back to school in 2017 where she received her masters degree in nursing specializing in family medicine. Since graduating as a family nurse practitioner, Sarah has worked in integrative medicine practicing functional medicine in primary care and specialty care including thyroid, BHRT, autoimmunity, tick borne disease, PCOS, MCAS, dysautonomia, root cause finding, and more. Sarah enrolled in The School of Applied Functional Medicine in 2021 where she is currently working towards her certification in functional medicine. Sarah believes in partnering with patients to achieve optimal health and avoiding disease processes before they even begin. Lifestyle changes, nutraceuticals, and herbs all can be used to reverse and prevent illness. Food is medicine. Outside the office, Sarah can be found caring for her 5 children and loving husband enjoying the outdoors, hiking, skiing, at church, baking, cooking, and studying. 

Sarah is currently accepting patients in primary care and for functional medicine evaluations with extensive testing. 

Austin McCaslin, DNP, APRN, FNP-BC

Austin graduated from Emory University with a BA in Psychology and from Sacred Heart University with a doctoral degree as a family nurse practitioner. Her interests are pediatrics, mental health, diabetes, and integrative/functional approaches to healthcare, including integrative approaches to treating autoimmunity, autism, ADHD, asthma, acne, lyme's disease, hormonal and thyroid imbalances, and PCOS. Austin is as an assistant professor at Goodwin University School of Nursing, specializing in pediatrics, and is currently enrolled in a PMHNP program at Spring Arbor University. She is a mother to four children and six chickens, active in church, and is a long-time member of the homeschool community. She believes in balance, natural solutions before pharmacologic solutions when possible, supporting families, and patient partnership. Austin is accepting patients of all ages for primary care and for mental health medication management, and works functional medicine into primary care appointments.

Erica Bencivenga, DNP, APRN, FNP-BC

Erica graduated from Southern Connecticut State University in New Haven, Connecticut with a bachelors in nursing. She joined the United States Navy Nurse Corps immediately upon graduation and served at Naval Hospital Jacksonville in Jacksonville, Florida and Branch Health Clinic Iwakuni in Iwakuni, Japan. While in the navy she was a labor and delivery nurse and was the clinic manager of a family practice Medical Homeport. Upon leaving the military and moving back to the United States, Erica went back to school in 2017 where she received her doctorate of nursing practice in family medicine from East Carolina University in Greenville, NC. In 2021, Erica and her family relocated back to Connecticut. Erica has worked in urgent care and pre-admission testing. She is passionate about breastfeeding, wellness for the family, reducing inflammation, nutrition, integrative and functional medicine. Movement and diet are foundational to preventing and managing disease. When Erica is not at work, she can be found spending time with her husband and three daughters, volunteering at church, homeschooling, baking bread, learning, and tending her garden and chickens. 

Erica is accepting primary care patients of all ages.

Rachel Arcila, DNP, APRN, FNP-C

Rachel graduated from Quinnipiac University in 2014 with her BSN.  She worked in adult and then pediatric intensive care as a registered nurse for five years while attending Quinnipiac University where she graduated in 2018 with her doctoral degree as a family nurse practitioner. 

She obtained her certification as a lactation counselor the same year.  She has specialized in pediatric primary care and providing lactation support as a nurse practitioner. Rachel has always sought to treat the whole patient, not just the diagnosis which is imperative to the functional approach to medicine.  Her favorite part of working with children is providing education to families.  She believes parents should be empowered and feel confident in the choices they are making for their children from sleep, nutrition, to normal developmental changes through a child's life. Rachel is passionate about using exercise and food as medicine, and is working towards completing the Solid Starts Professionals Curriculum for introducing solids to babies.  When she isn’t supporting families in the office, you can find Rachel enjoying the outdoors with her husband and two young children- gardening, hiking, or just playing outside.  She enjoys including her sons in everyday ‘normal’ activities like prepping and cooking meals, grocery shopping, and visiting their favorite bakery.  Rachel is currently accepting patients for primary care as well as those interested in lactation counseling and support.

We recognize the benefits of natural approaches in healthcare, that food can both heal and harm, and that the human body is capable of incredible healing when appropriately supported.

Primary care for all ages

Pediatric through geriatrics patients are accepted. We participate with most major insurance groups including Husky. We have a $30 new patient fee and a $30 yearly fee for patients who do not have Husky insurance.  The best way to become a new patient is signing the waitlist below. 

Family Centered Care

We believe the family unit is a blessing. We are here to support and nurture families!

Patient Partnership

We believe the best outcomes happen when providers and patients work together to support patient specific outcomes and goals. Ultimately you call the shots when it comes to your health! We are here to help you understand risks, benefits, and evidence supported options. We do not believe in "one size fits all" healthcare.

Services and Practice Information

We offer several levels of care and pay models 

 Extensive Testing and Collaboration

Functional medicine extensive testing: initial 1 hour appointment $200 follow up visit $75-100. Specialty testing review $75

Functional Medicine extensive testing visits are not covered by insurance. If you are happy with your primary care, we can work with you and your primary care provider to optimize health. 

Integrative Primary Care

We offer primary care for all ages and participate with all major insurance companies. We also offer a private pay model for primary care.
